چکیده
Introduction Diabetes Mellitus is a chronic illness that increases the risk of microvascular complication and results in blindness, kidney failure, and lower extremity amputations, and macrovascular complication of coronary artery disease and stroke. In the State of Hawaii, an estimated eight to ten percent of the population have diabetes mellitus, i.e. about 80,000 to 100,000 individuals, primarily adults with type 2 diabetes. Alarmingly, native Hawaiian adults over 30 years of age have been identified with a diabetes prevalence rate of 20%. Several studies clearly document the increasing rate of diabe tes among native Hawaiians over the past 30 years. Based on recent data, it is estimated that about 20,000 adult native Hawaiians (1820% prevalence rate) have diabetes mellitus) It would appear that interventions to prevent diabetes and diabetic complications receive the highest priority in the effort to improve the health status of the people of Hawaii, especially native Hawaiians. Two major intervention trials have clearly demonstrated that intensive glycemic control prevents and delays the development of microvascular complication in patients with type 1 diabetes: the Diabetes Control and Complications Trial (DCCT) and the Stockholm Diabetes Intervention Study (SDIS). 23 Results from a randomized prospective 6-year study in Japan show that intensive insulin treat ment of type 2 diabetic patients reduces microvascular complica tion. 4Many studies have demonstrated the association of hypergly cemia with macrovascular complications of ischemic heart disease and stroke and increased cardiovascular disease risk factors, e.g., dyslipidemia, hypertension, and obesity. The 12-year cardiovascu lar mortality of men screened in the Multiple Risk Factor Interven tion Trial (MRFIT) showed that diabetic patients consistently had higher cardiovascular mortality than non-diabetic men matched for one or more of the major risk factors for CVD. 5 The data from the

Implications of the United Kingdom prospective diabetes study.
The United Kingdom Prospective Diabetes Study (UKPDS) is the largest ever study to clarify the role of glycemia and blood pressure control in improving morbidity and mortality in patients with type 2 diabetes. The study examined the impact of glycemia and blood pressure control on microvascular and macrovascular risk, and if any particular therapy was advantageous for these patients. This artic...

The United Kingdom Prospective Diabetes Study (UKPDS) is one of the longest and largest clinical trials ever conducted. It explored the effects of intensive blood glucose and blood pressure control on the development of complications in patients with type 2 diabetes. Patients took part in this trial for up to 20 years and the drop-out rate was extremely low. The aim of this discussion paper is ...
متن کاملHypertension and diabetic retinopathy--what's the story?
Improved understanding of the role of hypertension in the pathogenesis of diabetic retinopathy presents both a challenge and an opportunity for ophthalmologists and other diabetic healthcare professionals to improve patient care. Around 40% of patients with type 2 diabetes are hypertensive, the proportion increasing to 60% by the age of 75. Recent reports from the United Kingdom Prospective Dia...
متن کاملUnited Kingdom prospective diabetes study follow-up studies establish a legacy effect of therapy for hyperglycemia but not hypertension.
In type 2 diabetes, early intervention with intensive glucose control has long-lasting effects, according to 10-year follow-up data on a cohort of patients with type 2 diabetes enrolled in the United Kingdom Prospective Diabetes Study (UKPDS). This was the conclusion of the 10-year follow-up study of the UKPDS, which was presented on September 19, 2008, at the 44th Annual Meeting of the Europea...

متن کاملThe UKPDS: a diabetes nursing perspective
Blood glucose control Improved blood glucose control in type 2 diabetes substantially decreased the risk of microvascular complications (retinopathy and nephropathy), but not macrovascular disease (e.g. myocardial infarction, stroke). There was no difference in overall risk reduction among the different intensive treatment groups, suggesting that improved glycaemic control was the principal fac...
